This book analyses the established model for clinical trials, identifies its weaknesses in evaluating new drugs and reexamines the aims. It makes a case for retaining its original strengths but in exposing the limitations of a method now more than 50 years old, indicates how the model must change in order to accommodate the new problems confronting drug developers and clinicians who must now treat a different population of patients than confronted them in the early 1960?s. The author presents two new models for the evaluation of putative antidepressants. They are based partly on a reconceptualization of the depressive disorder, itself, noting, that it is now viewed as a ?dimensional? disorder, best characterized as the interaction of disturbance in such psychological components as anxiety, anger, and cognition. This approach is counter to the traditional clinical concept of the disorder as singular with a core syndrome. The dimensional view requires measurement that goes beyond  evaluation of overall severity  to provide a profile of psychopathology, changes in components that can be measured with the application of new treatments. The chapters characterize the new componential and video models of evaluation, the methods involved, and compare their effectiveness in earlier work, with that of the traditional trial. The results clearly demonstrate superiority both in the evaluative procedure and in the markedly reduced expense required to conduct them. The book provides appendices that include examples of their application to average clinic patients, how the methods are applied, are scored, and how change in the profiles induced by a new treatment is recorded.
