Synopsis "Spitz-Tumore (in German)"

Spitz-Tumoren stellen ein Spektrum von Läsionen dar, zu dem Spitz-Nävi (NZ), atypische Spitz-Tumoren (AST) und spitzoide Melanome (MS) gehören. Die Unterscheidung ist manchmal schwierig und erfordert zusätzliche Techniken. Unsere retrospektive Studie umfasste 26 Fälle von Spitz-Tumoren. Neunzehn Patienten hatten NS, vier Patienten hatten ASD und drei Patienten hatten MS. Das Durchschnittsalter bei der Diagnose betrug 22 Jahre für NS, 10 Jahre für ASD und 33 Jahre für MS. In der NS- und ASD-Gruppe gab es eine leichte weibliche Dominanz (SR 0,46 und 0,33). Der durchschnittliche Läsionsdurchmesser war bei 95% der NS 10 mm. Histologisch gesehen waren die NS vom zusammengesetzten Typ (53%), rein dermal (42%) und vom junktionalen Typ (5%). Die TSA waren dermal (3 Fälle) und zusammengesetzt (1 Fall). Die MS waren maximal 1 bis 5 mm dick mit Clark II bis III. Die wichtigsten diagnostischen Kriterien für Gutartigkeit waren in allen Fällen von NS vorhanden. P16 wurde in 11 Fällen von NS (84,61%), in 2 Fällen von ASD (n= 3) und in 2 Fällen von MS (n=3) exprimiert.HMB45 war in 12 Fällen von NS (92%), in 2 Fällen von ASD und in den Fällen von MS positiv.
